Which in-person engagement session would you like to attend? *

Please register for only one in-person event. In order to make sure people feel supported at the meetings, spaces are limited. We want to allow as many people as possible to share their voices.

This engagement session is full. To ensure the highest level of participation, our sessions have limited capacity. I understand the Federal Indian Hospitals Foundation engagement process is not affiliated with the Claims Process for compensation. Survivors and family members who require support with their claim may contact the Claims Administrator or a Claims Helper (ihsettlement.ca). There is no cost to Survivors and family members for this support.

I understand that the purpose of these facilitated in-person sessions is for Survivors and family members to provide their direct input to Dr. Mike DeGagné on the establishment, structure, design, and activities of the Foundation.

As these in-person sessions are for the purpose of seeking input on the Foundation, they are not set up as an opportunity for sharing my full experience at Federal Indian Hospitals. If it is important to me that the future Foundation gather stories from Survivors and family members, I will include this in my feedback during the engagement session.

I understand that while the engagement sessions will not be digitally recorded, there will be notetakers present who will be taking notes by hand, including the general themes, feedback, and recommendations to share with Dr. DeGagné for the Foundation Plan. They’ll keep their notes anonymous, but if I wish, I can ask them to have my comments linked to my name. I will be provided with a consent form to review and sign. My comments may then be used as part of a public report or Foundation Plan. I may withdraw this consent by contacting the Indian Hospitals Foundation Engagement Support Team at any time before a final report or the Foundation Plan is published.
