Online Submission Form

For those individuals who cannot or do not wish to attend an in-person event, Dr. DeGagné still wants to hear from you through the submission form below. He is looking for direct advice, guidance, and comments from Survivors of Federal Indian Hospitals included in the Settlement Agreement and their family members and communities. Specifically, he wants to focus on input that will help shape the Foundation, including: healing, reconciliation, commemoration, research, and education.

Dr. DeGagné will collect all feedback, including what’s shared through the website submission form, and use it anonymously (unless express consent is sought to attribute information to a participant) to help shape the Foundation Plan. This way, the final recommendations reflect the voices and ideas of everyone who took part in the engagement process.

Any information submitted below will be kept securely in a database on a private server, based in Canada, owned and maintained by Indigenous data stewards. Your information will be held in compliance with frameworks such as OCAP: Ownership, Control, Access, and Possession; FAIR: Findable, Accessible, Interoperable, and Reusable; and CARE: Collective Benefit, Authority to Control, Responsibility, and Ethics; and the United Nations Declaration on the Rights of Indigenous People. Your name and accompanying information will be kept private and not shared with anyone beyond Dr. DeGagné and the Engagement Support Team who will respect your privacy throughout the entire process. Dr. DeGagné will reflect all the input he receives by the end of the engagement process in the Foundation Plan, but your name will never be shared publicly unless you expressly tell us otherwise.
